I believe a fetish is almost a clinical definition where a person cannot orgasm at all unless the fetish is present.

Of course, casual usage would be different. People without a clinical fetish would say they have a fetish, when it’s really just a sexual interest or a huge turn-on.

The clinical condition would be a lot harder to get rid of than a turn-on.
